Forms for Dupuytren Documentation

These forms are for documentation of clinical Dupuytren disease. Diagrams in the Office Visit form are oriented for the examiner across the exam table from the patient, whose hands are resting palms up on the exam table. The Zoom Visit form is oriented for the patient holding their hands up, palms facing the camera. All forms orient the patient facing the examiner: the right-hand diagram is on the left side of the form, and the left-hand diagram is on the right side of the form.
